Efficacy and Safety of Motilitone in Migraine Patients with Gastrointestinal Symptoms

Abstract
Background: Nausea and vomiting are a frequent accompaniment of migraine and prokinetic medications are frequently used in its management. Most prokinetic medications that are used in migraine are dopamine antagonists and therefore have the potential to cause drug induced extrapyramidal symptoms. We evaluate the efficacy and safety of motilitone, a recently developed prokinetic drug, in migraine patients with gastrointestinal symptoms.

Methods: From two outpatient neurological clinics, we prospectively included 100 migraine patients with nausea and vomiting and treated them with medications including motilitone. For the evaluation of safety, we used drug induced extrapyramidal symptoms (DIEPS) questionnaire, and the clinical improvement was assessed with clinical global clinical impression of change (C-GIC) and patients¡¯ global clinical impression of change (P-GIC).

Results: Among the 100 patients, only two patients described the presence of mild extrapyramidal symptoms (one with difficulty in standing up from chair and the other with difficulty in find movements), but it was not certain that these extrapyramidal symptoms were associated with motiltione treatment and both patients refused to dis continue motiltione with the clinical improvement. The mean values of C-GIC and P-GIC were 3.26¡¾0.8 and 3.19¡¾ 1.1 respectively, which indicate mild improvement with the treatment.

Conclusion: Our study shows that medication including motiltione for migraine patients with gastrointestinal symptoms is effective and there is low risk of clinically significant extrapyramidal symptoms. Further studies are necessary to accurately evaluate the risk of extrapyramidal symptoms with motilitone treatment in patients with migraine.
